Drugs giants AstraZeneca and other pharmaceutical companies are putting their rivalries aside to collaborate on a research program to tackle superbugs such as MRSA which can prove resistant to antibiotics. The firms will work with public partners on the initiative, which forms part of a European Commission action plan launched last November. It aims to boost the current faltering discovery and development of new antibiotics. The research programme’s first projects will be funded by up to £180m, provided by the companies involved – Astra, GlaxoSmithKline, Janssen, Sanofi and Basilea Pharmaceutica – and the Innovative Medicines Initiative, a Europe-wide public-private operation. Antibiotic resistance is seen as an increasing worldwide health threat. Effective antibiotics are required for many recent medical advances, such as chemotherapy, but bacteria resistant to existing drugs have continued to emerge and research has diminished.

Deadly MRSA; Are You the Cause of the Spread of This Killer Disease?

What role do you play in the spread of MRSA?

Is MRSA infection contagious? It can spread from person to person in a variety of ways – but most often via skin to skin contact and especially via the hands. MRSA is a highly drug resistant variety of the very common staphylococcus aureus (SA) bacteria.

SA is found on the skin of over 30% of people and is believed to be carried by as many as 80% of people at some point in their lives. MRSA is rarer – 1.5% are thought to carry it, but in some populations this can be as high as 10% and it seems likely to grow.

This week a judge told FDA to reverse course regarding antibiotic overuse in livestock and protect the effectiveness of the medicine for humans.

For the past 35 years the FDA has supposedly been reviewing the routine use of antibiotics in animal feed when in fact the agency was simply sitting on its hands. Then, last December, the agency caved to pressure and quietly withdrew any attempt to require the removal of antibiotics from animal feed.

Back in 1977, FDA concluded that feeding animals low doses of certain antibiotics used in human medicine, namely, penicillin and tetracyclines, could promote antibiotic-resistant bacteria capable of infecting people. Despite this conclusion—and in direct violation of laws requiring that the agency move on its findings—FDA failed to take action for the next 35 years.

The so-called “preventive” use of antibiotics in livestock is routine and widespread—80% of all antibiotics sold in US go into farm animal feed. Factory farms use them to ward off illness in animals that are kept in overcrowded, filthy living conditions, which are a perfect environment for the spread of illness. These antibiotics are also used to promote increased growth in animals.

However, antibiotics given to animals are transferred to humans through direct contact, environmental exposure, and the consumption and handling of contaminated meat and poultry products—making humans vulnerable to antibiotic-resistant superbugs, which are now a global problem. The judge also ruled that FDA must warn drug makers that the government may soon ban the agricultural use of antibiotics for animals that are not sick.

“For the foregoing reasons, Plaintiffs’ Motion for Summary Judgment on their first claim for relief is granted and Defendants’ Motion for Summary Judgment is denied. Defendants are hereby ordered to initiate withdrawal proceedings for the relevant NADAs / ANADAs (approvals to use antibiotics).

Specifically, the Commissioner of the FDA or the Director of the CVM must re issue a notice of the proposed withdrawals (which may be updated) and provide an opportunity for a hearing to the relevant drug sponsors; if drug sponsors timely request hearings and raise a genuine and substantial issue of fact, the FDA must hold a public evidentiary hearing. If, at the hearing, the drug sponsors fail to show that use of the drugs is safe, the Commissioner must issue a withdrawal order.”

Who does the FDA protect, anyway?What exactly is the Court ordering the FDA to do?

Although the Court is ordering the FDA to complete mandatory withdrawal proceedings the relevant penicillin and tetracycline NADAs/ANADAs, the Court is not ordering a particular outcome as to the final issuance of a withdrawal order. If the drug sponsors demonstrate that the subtherapeutic use of penicillin and/or tetracyclines is safe, then the Commissioner cannot withdraw approval.

Livestock farmers in the United States are coming under increasing pressure to cut down on the non-therapeutic use of antibiotics, after experts revealed that antibiotic resistance could aid the evolution of superbugs such as MRSA in humans.

Speaking at a science conference in London on Monday 26 March, Prof Lance Price, lead author a study into the use of antibiotics in livestock, said genome sequencing had provided “rock solid” evidence that antibiotic resistance had contributed to the emergence of the superbug MRSA.

EU ban

The EU banned the use of antibiotics as growth promoters in 2006, but US producers still use them widely, for growth promotion and to compensate for over-crowding and unsanitary conditions, said Erik Olson, Director of Food Programs of Pew Health Group.  At least they care about consumers in Europe!

In the US it is estimated that livestock producers use 300mg of antibiotics for every kg of meat produced, compared to UK producers, who only use approximately 115mg/kg.

“By aggressively controlling antibiotics in food animal production and human medicine we can ratchet back resistance considerably,” said Prof Price.

The experts admitted the US was significantly behind Europe and warned that its failure to address the issue immediately could have implications on trade in the future.

Maryland and Calvert County health officials didn’t report any new details about the cases, but the Washington Post reported yesterday that the sister of the 81-year-old woman who died has been hospitalized at MedStar Washington Hospital Center with fever but no other flu symptoms.

Yesterday the Maryland Department of Health and Mental Hygiene (MDHMH) said seasonal H3N2 influenza had been confirmed in two of the four initial illnesses, which involved the older woman and three of her adult children, two of whom died on Mar 5 after caring for their mother. The second daughter who cared for the woman is recovering at the same Washington, DC, hospital.

Hospital officials have said the patients’ influenza infections were complicated by co-infections with Staphylococcus aureus pneumonia prior to hospitalization, and Calvert County Health Officer David Rogers, MD, told the Post yesterday that the staph infections probably involve methicillin-resistant S aureus (MRSA).

Rogers also told the Post that the mother had been vaccinated against seasonal flu but that the children who cared for her had not.

Tom Skinner, a spokesman for the US Centers for Disease Control and Prevention (CDC), said the CDC was expecting autopsy samples from Maryland to arrive today, and the agency hopes to confirm what pathogens were involved within a few days, according to a USA Today report.

Secondary S aureus pneumonia is a potentially fatal complication of influenza, and one of the hallmarks can be severe necrotizing pneumonia, according to a report on 10 MRSA pneumonia cases, six of them fatal, reported to the CDC from Louisiana and Georgia during a 2-month span in the 2006-07 flu season. The report was published in the Apr 13, 2007, issue of Morbidity and Mortality Weekly Report (MMWR). The patients included both children and adults, and many were previously healthy.

The report details a short interval between respiratory symptom onset and death or isolation of MRSA from the patient. In four of the six fatal cases the patients died within 4 days of symptom onset, which the authors said suggests concomitant flu and MRSA infections, rather than a two-phase pattern seen with other types of flu and pneumonia.

Calvert Memorial Hospital, which originally treated some of the patients, said today that it has fielded a number of calls from worried area residents. Paul Pomilla, MD, the hospital’s medical director for infectious diseases, said in the statement, “We understand that many in the community are nervous about the flu after reading media reports about this family’s tragedy, but we want to reassure the public that we are not aware of any other cases of serious influenza-like illness that have a confirmed link to the original patients.”

The hospital advised families to follow routine flu prevention methods and to be vaccinated against flu if they haven’t been already, given the flu season’s late start. Hospital officials said they would continue to monitor flu-like illness patterns seen in the emergency department, urgent care centers, and hospital.

MRSA Can ‘Morph’ into Pneumonia

A flesh-eating form of pneumonia that is easily passed between healthy people on public transport is spreading across the UK and Europe from the United States, experts have warned.

The deadly strain of MRSA called USA300 passes easily through skin-to-skin contact. It can also survive on surfaces and so has the potential to be picked up on crowded buses, taxis and airliners.

USA300 is resistant to treatment by several front-line antibiotics and can cause large boils on the skin. In severe cases, USA300 can lead to fatal blood poisoning or a form of pneumonia that can eat away at lung tissue.

‘These community-acquired strains seem to be good at affecting healthy people – they seem to be much better than the hospital ones at causing disease. ‘They don’t rely on healthcare workers moving them around, which the hospital ones seem to.’

Chris Thomas, professor of molecular genetics at the University of Birmingham, said: ‘The key message is that strains of MRSA that are spreading in the community are better able to infect the young and healthy, precisely because they are not actually trying so hard to be resistant as the bugs that have been encountered in hospitals for many years.’

He said there was now a ‘need to worry about community super bugs that are fine tuned to spreading outside of hospitals and we all need to be extra vigilant about hygiene and unnecessary use of antibiotics.’
